Women who suffer from severe migraines are at increased risk of stroke and heart attacks, as a study has found it is the second most decisive factor after high blood pressure.

Migraine accompanied by seeing flashing lights or other vision problems was a stronger predictor of heart attacks and strokes than obesity, diabetes, smoking or family history, the researchers said. Previous studies have suggested that migraine with aura, as the vision disturbances are known, is linked with a doubling of the risk of a heart attack or stroke. Dr Tobias Kurth, lead author, said it is not yet known if the migraine with aura causes the increased risk or if there is a common factor causing both the migraine and the heart and stroke. He said it might be that a problem with blood vessels lay behind both, that genetics were involved along with other factors. He said: "Among doctors it is not currently our reflex when we see a patient with migraine with aura to ask about their heart disease risk. We need to focus on this a little bit more."Most risk factors can be treated, such as high blood pressure, smoking, diabetes and high cholesterol. If someone has migraine with aura they may want to look at these factors as a way of reducing their risk of a heart attack or stroke." Dr Kurth added that even though the risk for migraine with aura sufferers was increased, it was still a minority of them who had heart attacks and strokes.
There was no elevated risk for people with migraine without aura. He added that although the study was conducted on women there is no reason to believe the results would be any different in men. The research to be presented at the American Academy of Neurology meeting in San Diego looked at almost 28,000 women aged over 45 who were followed up for 15 years. Of those 5,130 suffered migraines, 40 per cent of whom also had aura. It was found that there were 2.4 heart attacks or strokes per 1,000 women per year in the whole group. The women with high blood pressure suffered the most heart attacks and strokes with 7.9 per 1,000 women per year, followed by women with migraine and aura with 7.9 heart attacks and strokes per 1,000 women per year. This was greater than women with diabetes who suffered 7.1 heart attacks or strokes per 1,000 women per year, family history of heart attacks with 5.4 per 1,000 women per year and current smokers who suffered 5.4 heart attacks or strokes per 1,000 women per year. Women who were seriously obese suffered 5.3 heart attacks and strokes per 1,000 women per year. Around seven million people suffer with migraine and 700,000 also have aura. Susan Haydon of The Migraine Trust said: "It is known that young women with migraine with aura have a slightly greater risk for blood clots than similar young women without migraine or with migraine without aura. "Therefore the practical implications are to reduce risk by not smoking, also combined hormonal contraception is generally not suitable for young women with migraine with aura due to the increased risk of ischemic stroke. "The risk is very small, as stroke is rare in young people. It is not thought that migraine with aura causes the stroke, but rather that migraine aura is a marker for young women at a greater risk. "Migraine is considered to be insignificant as a risk factor for stroke after the age of 50 years. "This is because the usual risk factors for ischemic stroke are high blood pressure, obesity, raised blood cholesterol levels, smoking and older age. These factors tend to combine with each other and, with advancing age, the risk of stroke due to migraine becomes insignificant in comparison with the other risk factors."
